On the portal homepage, locate and click the Buy Branch Nomination Forms button. You will be directed to the purchase form page.

Do not use browser back/forward navigation during this process. Verify the URL remains forms.ghanandc.com before entering any details.

Complete the Purchase Form
Fill in all required fields accurately — incomplete entries will prevent submission. Enter your Mobile Money (MoMo) number carefully as this processes your payment.

Double-check your name, contact details and branch information. Fields marked (*) are mandatory.

Bulk Purchase (If Applicable)
If purchasing forms for multiple people, toggle the Bulk Payment option. Enter the quantity desired — the total payable amount is calculated and displayed automatically.

Confirm the quantity before proceeding to the payment step.

Approve MoMo Payment
Review the payment summary displayed on screen. You will receive a Mobile Money prompt on your registered phone — approve the transaction to complete payment.

Do NOT close the browser while the MoMo approval is pending. If the prompt does not arrive within 60 seconds, check your network and try again.

Receive Your PIN & Access Code
Upon successful payment you will receive an SMS and/or email containing your unique Access Code and PIN. Keep these strictly confidential.

NDC staff will NEVER request your PIN or Access Code. If the message doesn't arrive within 5 minutes, contact the helpdesk via the portal.

How much does the Branch Nomination Form cost?

The Branch Nomination Form costs GH₵20, payable via Mobile Money (MoMo). This small fee covers administrative processing. If purchasing in bulk for multiple people, the total is calculated automatically based on quantity.
What if I don't receive my PIN via SMS?

If your PIN/Access Code SMS does not arrive within 5 minutes of successful payment, first check that your registered phone number is correct and that you have network reception. Then contact the NDC Forms helpdesk directly via the portal at forms.ghanandc.com.
Can I edit my form after submitting it?

No — once you click the final Submit button, your nomination form is locked and cannot be edited. Please review all four sections carefully using the Review (Section 4) step before submitting. If you have made an error after submission, contact the helpdesk immediately.
What is a Redemption Code (Coupon)?

A Redemption Code (also called a coupon) is a unique code sent to someone who had their form purchased on their behalf — for example, by a coordinator buying in bulk. If you received such a code, skip the purchase steps and go directly to Phase 4 (Claim Your Form) on this guide.
What documents do I need to upload?

You will need to upload all required supporting documents in Section 3 (Attachments). Accepted file formats are PDF, JPG, and PNG. Ensure each file is legible, correctly labelled, and within the portal's specified size limit. Check the portal instructions for the current list of required documents.
Is it safe to pay via Mobile Money on this portal?
